Summary: The Order Closure Specialist works closely with the Referrals team, Risk Management, Quality, and Health Information Management Departments.
The primary responsibility of this position is to be liaison between MLMC and the patient to ensure that orders placed by the Provider are completed and supporting documentation is returned to MLMC in a timely manner.
This position also ensures the closure of Value Based metrics and addressing gaps in care.
Essential Duties and Responsibilities: Monitoring and tracking of referrals and test results: A.
Responsible for monitoring and following up with patients regarding incomplete referrals to ensure receipt of results in the Electronic Health Record (EHR) and specialist notes for patient safety across all MLMC locations.
B.
Monitoring and follow up with patients regarding incomplete lab and diagnostic testing orders ensuring tests are completed and documented in the EHR.
C.
Assesses for any barriers that are preventing order completion.
Connects patients to the appropriate resources to address barriers such as transportation or other social determinants of health.
D.
Monitor loop closure and compliance rates among the patient population E.
Tracking of no-show, cancellations, and referrals never scheduled or rescheduled if necessary.
Documentation Standards: A.
Ensures documentation is noted in the system of the resolution of the ordered tests.
There must be documentation in the medical record of the number of and methodology of attempts to reach the patient.
B.
Identify other practices, where there may be opportunities for bi-directional interface and documentation sharing protocols with area specialists and providers.
C.
Maintain compliance with HRSA and MLMC referral and testing standards, policies, and procedures in accordance to MLMC policies CL201-CL204.
Value-Based Gaps in Care: A.
May be asked to address gaps in care as generated by the EHR or the payer gaps in care reports.
Addressing these gaps may require phone calls to the patient, sending correspondence via the patient portal or by letter, or appointment coordination.
B.
Document and track efforts of outreach to patients regarding the patient insurance, test needed, order placed, and appointment made.

Qualifications: The successful candidate will have a demonstrated work record showing good attendance, punctuality, dependability, and the ability to work as a member of a team with supervisors, providers, and other co-workers.
The candidate will also have strong computer skills including familiarity with spreadsheet programs.
Education and Experience: High School Diploma or equivalent, and basic medical terminology required.
Experience with order management and with spreadsheets strongly preferred.
Electronic Health Record and appointment scheduling experience preferred.
Must maintain good phone, communication, and customer service skills with the MLMC patient population and our Community Partners.
Computer Literacy: Knowledge of basic computer software and the willingness and ability to learn EMR software.
Work Record: A demonstrated work record showing good attendance, punctuality, dependability and the ability to work well with supervisors and coworkers as part of a team effort is essential.
A medical office setting is an environment that requires the ability to relate to a diverse population while always maintaining a professional demeanor.
Language Skills: Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals is essential.
Ability to write routine reports and correspondence, and the ability to speak effectively to patients, co-workers and the public at large is of the utmost importance.
Mathematical Skills: Must have the ability to calculate figures and amounts such as, proportions and percentages along with mathematical accuracy and attention to detail are essential skills for this position.
Reasoning Ability: Must be able to demonstrate the ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
Team Work: An essential element is the ability to work and interact effectively and positively with other staff members to build and enhance teamwork in a patient centered team based model of care and in the overall MLMC organization; team engagement is a must to be successful in this position.
