Holbrook Healthcare Center, a member of the CommuniCare Family of Companies is currently recruiting for a Transportation Aide, CNA Required to join our team.
PURPOSE/BELIEF STATEMENT
The position of Transportation Assistant exists to assist residents with transportation to and from inside facility and outside physician appointments and events in a safe manner and in accordance with established procedures.
Candidates must have a clean driving record and, as a Certified Nursing Assistant, must be able to lift and transfer residents.
The Transportation Aide must be willing to work a variety of hours and weekends depending on the need.
JOB DUTIES & RESPONSIBILITIES
Transport and assist residents, per facility vehicle, to outside physician visits in a safe manner.
Assist with transporting residents within the facility to scheduled specialty appointments, i.
e.
podiatry, audiology, optometry, dental, etc.
Assist with transporting residents to and from scheduled in-house activities.
Provide one on one monitoring for residents deemed an elopement risk on outside visits.
As a CNA:
Transfer and lift residents.
Perform basic nursing care procedures for residents as required.
Create a clean, comfortable, and safe environment for residents and assure that their needs are met.
Maintain resident documentation records.
Maintain and work within established departmental, center, and corporate policies and procedures, objectives, quality improvement program, and safety, environmental, and infection control standards.
Attend and participate in scheduled training, educational classes, and orientation programs to maintain certification and enhance quality of care.
Perform other related activities as assigned or requested.
QUALIFICATIONS & EXPERIENCE REQUIREMENTS
High school graduate or G.

D.
equivalent
Must possess a current, valid Driver’s License in State of employment.
CNA required: Graduate of an approved Nurse Aide Training Program or be eligible to enter such a program and graduate and pass a state test within 120 days from date of employment.
Must possess Red Cross First Aid Certificate.
