As a Stonerise Home Health RN, you will plan, organize, and direct home care services with an emphasis on community health education and experience.
You will build from resources of the community to plan and direct services to meet the needs of patients and families within their homes and communities.
Some of your primary responsibilities include:
Completing an initial assessment of patient and family to determine home care needs.
Providing a complete physical assessment and history of current and previous illness(es)
Supervising and directing the Licensed Practical Nurse (LPN) and/or the Home Health Aide in implementing the patient care plan
Developing patient education plans, as needed, and directing the LPN in implementing said plans
Creating and modifying the Home Health Aide care plan and supervising them via patient feedback and onsite interactions
Evaluating and revising plan of care as patient status and needs change
Using health assessment data to determine nursing diagnosis
Developing a plan of care, which establishes goals based on nursing diagnosis and incorporating therapeutic, preventive, and rehabilitative action.
Administering medications and treatments as prescribed by the physician
Identifying discharge planning needs as part of the care plan development and implementing prior to discharge of the patient
Acting as the Case Manager when assigned by the Clinical Supervisor and assuming responsibility to coordinate patient care for assigned caseload
Preparing clinical notes as needed and communicating with the physician regarding patient’s needs

Non-Exempt
Education, Licensure & Certification Requirements
Graduate of an accredited school of nursing.
Active RN licensure
One (1) to two (2) years of recent acute care experience in an institutional setting
Bachelor’s degree with one (1) year of home health care experience preferred
Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
