The LPN will serve as an important member of the treatment team for individuals with substance use disorders.
The LPN will be responsible for medical assessments of all patients during intake and will meet with active patients on a regular basis to address their medical needs.
The LPN will work closely with other team members on a wide variety of administrative and operational tasks to ensure the facility runs properly.
Accountabilities Complete medical assessments for patients during intake process, including interviewing and annotating patient medical history, taking vitals, and performing drug screenings among other responsibilities.
Oversee confidential health risk assessments and access to counseling and testing.
Educate patients on medication adherence and additional health information relevant to their specific care plans.
Support daily operations of facility, including assisting with front desk inquiries and other administrative or operational duties.
Generate and review monthly medication administration records (MARs) or reconciling them to MARs provided by a duly authorized and qualified pharmacist or pharmacy.
Promote and adhere to the workplace values of integrity, respect, trust, teamwork, quality, and excellence.
Education and Experience, Certificates, and Licenses: West Virginia Licensed Practical Nurse Comfortable working independently with the population when needed Compliance with MAT (Medication-Assisted Treatment) program protocols Experience working with substance use disorder preferred.
Ability to work across teams and functions, self-starter who quickly identifies what needs to be done and takes ownership.
Multi-tasking capabilities in delivering clinically excellent care.
CPR certification required/must be obtained within 30 days of hiring.
