West Virginia University Research Corporation is seeking applications for a (Hybrid) Substance Use Disorder (SUD) Care Navigator with the Department of Behavioral Medicine & Psychiatry
About the Opportunity
The SUD Care Navigator provides community-based and office-based care management services to clients with substance use disorder (SUD) of the Department of Behavioral Medicine and Psychiatry who have been identified through clinical assessment to have a higher need for care management type resources for advocacy, linkage, referral and connected collaboration to support improvement in their daily life.
The SUD Care Navigator will be the main point of contact for identified clients.
The SUD Care Navigator builds strong relationships with clients in order to help clients stay engaged in medical care, adhere to their medications, and improve overall patient outcomes.
Care Navigators are committed to removing the client’s barriers to care by identifying critical resources for clients, helping them navigate through health care services and systems, and promoting client health.
They work closely with the Care Team, which may include doctors, nurses, and other clinical staff to support positive client health outcomes.
This position works with the clients to provide warm handoffs and work through social determinants of health barriers that they may have.
They will be working collaboratively as a team to improve patient outcomes along the entire continuum of care.

What you’ll do: This position will be a combination of community based and office-based work depending on the needs of the patient.
Maintain a professional relationship with patients, their families, community, treatment team, and other professionals.
Accompany clients to medical appointments, to develop that relationship and warm handoff.
Communicate with Care Team members (SUD treatment team, Care Coordinators, primary care providers and other health care providers) to facilitate client care coordination Assess patients and families for service needs.
Community-based care navigation includes Identifying and assisting with resources for clients to overcome barriers to care and basic needs.
Such as transportation, housing, childcare arrangements, healthcare, food, employment, financial, etc.
Develop individualize and/or family, strength-based service plan that is measurable with targeted outcomes.
Remain aware of current services offered by service providers in the community.
Maintain strict confidentiality in accordance with agency policies.
May meet with clients after appointments to review and update care plan with members of the treatment team.
Provide services based on Recovery Model.
Link patients to the recovery community including meetings, groups, classes, and others.
Organizational duties: Meet with Care Team (including, but not limited to, Care Coordinator and primary care provider) to discuss client care issues and needs and facilitate client health care.
Maintain documentation of all client encounters and complete reporting requirements according to organization standards.
Track client information, schedules, files, and forms in a confidential manner.
Track client attendance at medical appointments and patient navigation sessions and initiate outreach and missed appointment procedures, as necessary.
Track identified outcome measures.
Attend and represent the organization at professional conferences, in-service trainings, and meetings at the request of or with the approval of supervisor.
Extensive local travel Some evening and weekend work may be required Qualifications Bachelor’s degree in a Human Service Field or Similar Degree OR an equivalent combination of education and experience.
Two (2) years: Strong understanding of cultural competency with the target audience Experience with the SUD population High degree of creativity and self-motivation.
Substantial ability to work both independently and cooperatively with others.
Ability to manage multiple projects concurrently.
Ability to combine pieces of information to form general rules or conclusions.
Ability to recognize when something is wrong or is likely to go wrong.
Skilled at developing and managing relationships with diverse communities and individuals Experience and skilled in standard office computing and computer software applications (e.
g.
, Microsoft Office applications particularly including Outlook, Word, Excel, and PowerPoint).
Knowledge and understanding of addiction medicine.
Skilled at building and maintaining positive relationships with those involved in a program.
Skilled at operating effectively in a fast-paced, high-volume environment & meet deadlines.
Skilled in speaking effectively before groups of customers or employees of the organization.
Facilitating collaboration among agencies.
Strong organizational skills.
Excellent attention to detail, flexible and adaptable to change.
Able to identify deadlines and schedule work to ensure that deadlines are met.
Commitment to the mission of care coordination Passionate, trustworthy, and empathetic when working with clients.
